New Client Information

I accept most major insurance policies and am contracted as an "in-network" provider for BCBS, the NC State Health Plan and the UNC Student plan. For new clients, I will contact your insurance to determine cost required by your policy prior to our first meeting. To obtain this information, I will need your subscriber and group id # and your date of birth. Additional information may be needed if you are not the primary holder of this insurance plan. 

My standard rate is $195 for the Diagnostic assessment performed at the first session. Follow up visits are $170 for a 45-50 minute session. If you are using insurance, I honor the contracted rate designated by your plan. Payment will be expected at the time of service. I accept cash, check, and major credit cards (Visa, Mastercard, American Express and Discover). An additional 2.75% service fee is added to credit card charges. If you have financial constraints, I am willing to work out a payment plan with you. 

24 hours notice is required for cancellations. Otherwise, I am unable to offer the session to other clients. Thus, you will be expected to cover the cost of the missed session.

At this time, I am offering HIPAA compliant video sessions and will be returning to the office in June of 2022 on a part-time basis. My office is located on the 2nd floor of the Glen Lennox building in Chapel Hill, NC, at 1201 Raleigh Rd., Suite 202. My office is on the G, S, and V bus lines of the Chapel Hill Transit.

Please bring a copy of your current insurance card, method of payment, and the forms below. Once you arrive at suite 202, please make yourself at home in our waiting area, and I'll be out to greet you at our designated appointment time.
